Help improve the application process for ODSP

The Government of Ontario's Ministry of Children, Community and Social Services is making it easier to apply for the Ontario Disability Support Program (ODSP). We’d like your feedback to make sure the application process works for clients at every step.
 
ONE-ON-ONE SESSIONS
We are running one-on-one sessions to help us better understand a person’s journey in applying for ODSP benefits.
Sessions will be one-hour long and will take place online or by phone.

WE ARE LOOKING FOR FEEDBACK FROM:

Fill out this 5-minute SURVEY to see if you qualify for a one-on-one session.

The information you provide will only be used for the purposes of this project and will be handled in accordance with the Freedom of Information and Protection of Privacy Act.

Participating in a session
If you qualify for a one-on-one session, we will contact you in the next couple of weeks via phone or email (depending on your preference).

What you get:
You will receive an honorarium in the form of a $60 gift card to a local retailer. Only those people selected to participate in the session will receive an honorarium.
